What should I do about a sex problem?

Ok. Please no bullshit I'm serious. I am engaged to be married. My wife is a very beautiful small Asian woman. I am a large mixed Male. Intercourse is an issue. I have resulted to not asking for it anymore because I feel so guilty. She is the only one that will ask for it but I clearly know it is painful. This is really bothering me. I don't think I am oversized but I think maybe she is a bit small? Sometimes afterward she will bleed. This is upsetting to me as I would never want to hurt her. I am very conservative so I don't have much experience other than my ex-wife. That was the only other person I have been with and we had the same problem. I don't want to lose my current girl like I did my ex and I'm not sure what to do. I have asked two physicians and they both just laugh it off. It is not funny and for us this is serious. We want children we are both nearing our 40s and we want to have that intimacy. I bought her toys because I thought maybe if we had something to help "part the waters" then I could better visit without tearing the place down after? Just looking for advice. I am sure some trolls will comment but if a few of you who have serious comments and advice for us, it is much appreciated! The doctors have been no help.

My (23M) girlfriend (25F) relationship is confusing to me. I might be the problem, or maybe we are just incompatible.

I met my girlfriend after a few serious breakups that led me to change both intentionally and unintentionally over a few years. For better and for worse. She had just gotten out of an engagement with a verbally and physically abusive man who had a cocaine problem. We have been together about 8 months now.
