Partner gets headache after orgasming?

My partner (31M) will get a terrible headache after orgasming. We have sex for a bit, between 45 minutes to an hour. He has an autoimmune disease that deals with his spine, and also had a previously broken jaw from a car accident quite a bit ago. We have yet to determine why he gets these insane headaches where he’s near tears after it every time. We’ve either chalked it up to dehydration, holding in his breath, his autoimmune or the previous broken jaw. I think a lot of it is due to stress. I would like to say 9 times out of the month we have sex, at least 7 of them result in a headache or on the verge of the headache itself where he gets the tinge of it.

Has any one else had experiences with this? What has alleviated this, for you or your partner? I (32F) have only had 1 sex induced orgasm but it was never like his, and his are described as a “thunderclap”.
