How many chances should you give a 'oh something just came up' date?

So many dates and even friends keep hitting me with the 'oh something just came up I can't make it/I'm gonna be x hours late' too often and usually multiple times in a row, and sometimes season it with a little ghosting. Whenever I try confronting people like this, they often get very defensive and pin the blame on me. I do understand that shit actually happens, but I usually give them too many or too little chances, so I'm just wondering what you guys think is an appropriate number of chances to give a date/friend like this.

Also to the women reading, is this something women just do to test men? One time I told a date I'm no longer interested in her and don't want to see her anymore after she kept cancelling dates for 3-4 weeks straight (without reason usually), then she surprisingly got very upset.
